A $100 booking deposit is required at the time of booking and is applied toward your treatment or product at Revive.

Missed Appointments

We require a minimum of 48 business hours’ notice for any changes to your appointment. Missed appointments or changes made with less than 48 business hours’ notice are subject to a $100 fee.

Cancellation List
