Neuromodulators work by temporarily relaxing targeted muscle activity. In this case, small amounts may be placed into the orbicularis oris muscle, the circular muscle that surrounds the mouth and influences how the upper lip moves when speaking or smiling.
By reducing muscle tension in this area, the upper lip may sit or roll slightly outward, which can allow more of the lip to be visible at rest or during expression. This approach does not add volume to the lips, but instead affects how the lip moves and presents.

Comparing the two approaches
While both options relate to lip appearance, they address different concerns:
- Dermal fillers are typically discussed when volume, shape, or contour is the primary concern
- A neuromodulator lip flip may be discussed when lip movement or muscle activity affects how much of the lip is visible
